for the Referee Registration Form

Clearly on the form when you are filling it out!
When did you
start to referee? Year: ___
- When did you get
your Regional Level Certification.
- If you are from
outside of Canada, which year did you start to referee.
Certification
level?
- If you were
Provincial Level in the old system more than 2 years choose
Provincial A
- If you were
Provincial Level in the old system less than 2 years choose
Provincial B
- If you were
between Regional and Provincial Level and you refereed more than
a year choose Provincial C.
- If you were
between Regional and Provincial Level and you refereed less than
a year choose Regional.
- If you attended
the Regional Referee Clinic, but did not Referee more then 20
games, then do not choose anything.
Club
Affiliation?
- If you play and
coach on a different team, please write down both teams.
- If you play for
more then one team, please write down all of them.
Do you need
transportation?
If you do not have a car and you'd like to get help, mark YES so we
can organize to someone to give you a ride or pick you up somewhere.
Everyone who is
under Provincial A Level will have a temporarily Level until
December 31. 2004 and then the Referee Committee will decide each
referee level in the Province.
